Fill in the form with your details, which will be used to verify your attendance at the School. Once verified, you will receive a welcome email with your username and password.

• Enter your forename and any initials into the field marked "Forename" .
• Enter your surname into the field marked "Surname" .
• Enter your current, valid email address into the field marked "Email" . Your welcome email will be sent to this address so it is important that it works and you have access to it. The sender's email address will be from the domain ohdb.co.uk if it requires whitelisting.
• Select the School you attended from the field marked "School" .
• Please select the year you left the school, or, if you left before your peers, the year that your peers left, from the field marked "Year of leaving" .
• If you were a member of staff, please tick the box marked "Were you a member of staff?" .
• Finally, "select the picture of a cat" by using the selection buttons underneath the pictures. This is to prevent robots from submitting the form, as this is an easy task for a human to perform but extremely difficult for a robot.
• Click "Register" to submit the form. You will receive an email notifying you that your application has been received and once it has been verified you will receive another email to inform you of this and of your access credentials.

The School will ensure that you did indeed attend at the years you specified, and verify your account, of which you will be informed of by email. This keeps the database closed to members of the School only.
